Iron Panel: What Serum Iron, TIBC, Transferrin Saturation and Ferritin Results Mean

An iron panel is a group of blood tests that shows how much iron is circulating in your blood, how much your body has in storage and how well iron is being transported to where it is needed. It usually includes serum iron, total iron-binding capacity (TIBC), transferrin saturation and ferritin. Read together, these four results can separate iron deficiency from iron overload and from anemia caused by inflammation, three situations that a hemoglobin result alone cannot tell apart. One result outside the range is common; the pattern is what matters.

What an iron panel measures

Iron is needed to make hemoglobin, the protein in red blood cells that carries oxygen. The four core results

  • Serum iron: the amount of iron carried in the liquid part of your blood at the moment of the draw.
  • Total iron-binding capacity (TIBC): how much iron your blood proteins could carry if fully loaded, an indirect measure of transferrin.
  • Transferrin saturation (TSAT): the percentage of that carrying capacity actually filled with iron, calculated as serum iron divided by TIBC, multiplied by 100.
  • Ferritin: a protein that stores iron, mostly in the liver; the level in blood reflects your iron reserves.

Some reports also list transferrin directly or the unsaturated iron-binding capacity (UIBC). Why your doctor orders an iron panel

The most common reasons are tiredness, pale skin or shortness of breath, a low hemoglobin or small red cells on a blood count, heavy menstrual periods, pregnancy, a digestive condition that affects absorption and a family history of hemochromatosis. The ranges are wide because iron status varies a lot between healthy people. The key distinction is between a true lack of iron and iron that is present but locked away. In iron deficiency the stores are empty, so ferritin is low and TIBC climbs. Ferritin: the iron store gauge

Low ferritin

Serum iron, TIBC and transferrin saturation

Serum iron on its own is the least reliable member of the panel. MedlinePlus notes that the amount of iron in your blood varies throughout the day and may be higher in the morning, and a recent iron-rich meal or supplement can raise it for hours. That is why it is interpreted together with TIBC and saturation rather than alone.

TIBC reflects transferrin, the iron-carrying protein. Causes of high iron

MedlinePlus lists hereditary hemochromatosis, the breakdown of red blood cells in hemolytic anemia, liver damage and hepatitis, iron poisoning and frequent blood transfusions. Hemochromatosis is an inherited condition in which the gut absorbs too much iron over many years; it is usually suspected when transferrin saturation stays above 50% on repeat testing and ferritin is high, and it is confirmed with genetic testing. How to prepare for an iron panel

Recent research suggests that low iron is far more common than anemia statistics show, and that the choice of test changes who is counted as deficient.

Iron deficiency is common even without anemia

A 2024 analysis of more than 8,000 adults in a national US health survey found that about 14% had absolute iron deficiency, defined as ferritin below 30 ng/mL, and about 15% had functional iron deficiency, meaning normal stores but a transferrin saturation below 20%. Different tests give different answers

A 2025 analysis of US survey data from women aged 20 to 44 compared the tests commonly found in medical records with ferritin, the marker recommended by the World Health Organization. Among nonpregnant women, the share classed as iron deficient ranged from about 6% to 25% depending on which single test was used. In heart failure, saturation predicts more than ferritin

A 2025 study of 2,050 people with heart failure in Pennsylvania found that a low transferrin saturation and low serum iron were linked to a higher risk of death, while ferritin was not. Because inflammation raises ferritin, it can hide true iron shortage in chronic illness. What this means for you: if you have heart failure or another long-term condition, your doctor may focus on saturation rather than ferritin when deciding whether you need iron.

Rethinking the ferritin cutoff in early pregnancy

A 2026 study of 452 healthy women in early pregnancy in Vietnam looked at the ferritin level below which red cells start to change and suggested about 59 ng/mL, far above the usual cutoff of 15. The authors propose four zones, from absolute deficiency below 15 to sufficiency at 59 or more. These thresholds are preliminary and come from a single hospital. What this means for you: if you are pregnant or planning a pregnancy, a ferritin at the low end of normal is worth discussing with your care team.
